Fatigue is a subjective feeling of tiredness that differs from sleepiness. It can be classified into physical fatigue, related to muscle weakness, and mental fatigue, involving cognitive impairment. The underlying mechanisms are multifaceted, involving central and peripheral factors, and are often associated with various medical, psychiatric, and lifestyle conditions.
An effective clinical approach begins with a comprehensive history and physical examination, focusing on the duration, onset, pattern, and associated symptoms of fatigue. It is essential to explore potential triggers such as stress, sleep disorders, diet, physical activity, and substance use. Objective assessment tools, like fatigue severity scales, can also be utilized.
Investigations should be guided by the clinical evaluation. Routine tests may include complete blood count, thyroid function tests, and metabolic panel. Further investigations, such as sleep studies or neuroimaging, may be warranted based on the clinical suspicion of underlying conditions.
Management should be individualized, targeting the identified causes. This may range from lifestyle modifications, pharmacological interventions, to cognitive behavioral therapy. In cases where no specific cause is identified, symptomatic management and supportive care are crucial.
Fatigue is a complex symptom with a broad differential diagnosis. A comprehensive, patient-centered approach is vital for effective management. By understanding the multifaceted nature of fatigue, healthcare professionals can enhance their diagnostic accuracy and improve patient outcomes.
