Abstract

Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly known as nonalcoholic fatty liver disease (NAFLD), is rapidly becoming the leading cause of chronic liver disease worldwide. This review synthesizes recent evidence on the epidemiology, pathophysiology, clinical presentation, diagnostic modalities, and management strategies, emphasizing early diagnosis, lifestyle interventions, and emerging pharmacotherapies. Clinically relevant insights and current guideline recommendations are highlighted to support improved patient outcomes.

Introduction

MASLD encompasses a spectrum of liver pathology ranging from simple steatosis to steatohepatitis, fibrosis, and cirrhosis in individuals with metabolic dysfunction and absent secondary causes. As global rates of obesity, diabetes, and metabolic syndrome rise, MASLD has garnered increasing attention within hepatology and primary care. Early recognition and intervention are imperative due to the progressive nature and significant morbidity associated with advanced disease. This article provides an in-depth, evidence-based review tailored for healthcare professionals managing MASLD in diverse clinical settings.

Epidemiology / Disease Burden

MASLD affects approximately 25–30% of the global adult population, with considerable regional variation. Prevalence is higher among individuals with obesity (up to 70–80%), type 2 diabetes mellitus (T2DM), and metabolic syndrome. The shift from viral hepatitis to MASLD as a leading indication for liver transplantation in Western countries underscores its clinical significance. MASLD is also associated with increased all-cause and cardiovascular mortality, posing a substantial public health challenge.

Pathophysiology

MASLD is characterized by hepatic steatosis resulting from an imbalance between lipid acquisition and removal in hepatocytes. Insulin resistance, a core feature, promotes increased free fatty acid flux to the liver, augmented de novo lipogenesis, and impaired beta-oxidation. Inflammatory mediators, oxidative stress, lipotoxicity, and gut microbiome alterations further contribute to hepatocellular injury, promoting progression to steatohepatitis and fibrosis. Genetic polymorphisms, such as in the PNPLA3 gene, modulate susceptibility and disease severity.

Risk Factors

Major risk factors for MASLD include central obesity, T2DM, dyslipidemia, metabolic syndrome, and sedentary lifestyle. Age, male sex, postmenopausal status, and certain ethnicities (notably Hispanic populations) are also associated with increased risk. Additional factors—such as polycystic ovarian syndrome, obstructive sleep apnea, and hypothyroidism—may contribute. Genetic variants, family history, and environmental exposures play roles in individual susceptibility.

Clinical Features

MASLD is often clinically silent, with most patients remaining asymptomatic until advanced stages. When present, symptoms are nonspecific and may include fatigue, right upper quadrant discomfort, or malaise. Hepatomegaly is occasionally detected on physical examination. Advanced disease may manifest with signs of portal hypertension or hepatic decompensation, such as ascites, jaundice, or variceal bleeding. Importantly, MASLD is a systemic disease, increasing the risk of cardiovascular events, chronic kidney disease, and extrahepatic malignancies.

Diagnosis

Early diagnosis of MASLD relies on a combination of clinical suspicion, laboratory evaluation, and imaging. Elevated aminotransferases are neither sensitive nor specific but may prompt further investigation. Ultrasound is the preferred initial imaging modality, demonstrating increased hepatic echogenicity. Advanced imaging, such as transient elastography or MRI-based techniques, allows quantification of steatosis and fibrosis. Noninvasive scoring systems—such as FIB-4, NAFLD fibrosis score, and enhanced liver fibrosis (ELF) test—aid in risk stratification. Liver biopsy remains the gold standard for diagnosis and staging but is reserved for selected cases with diagnostic uncertainty or suspected advanced disease.

Treatment & Management

Lifestyle modification is the cornerstone of MASLD management. Weight loss of 7–10% has been shown to improve steatosis, inflammation, and fibrosis. Structured dietary interventions (e.g., Mediterranean or hypocaloric diets) and regular physical activity are recommended. Optimization of comorbidities—including glycemic control, hypertension, and dyslipidemia—is essential. Pharmacologic options are limited but include pioglitazone (in selected patients with NASH) and vitamin E (in non-diabetic adults with biopsy-proven NASH). Bariatric surgery may be considered for patients with severe obesity and comorbidities. Multidisciplinary care, including hepatology, endocrinology, and nutrition, optimizes outcomes.

Recent Advances / Emerging Therapies

Novel therapeutic agents targeting diverse pathogenic pathways are under investigation. GLP-1 receptor agonists (e.g., semaglutide) and SGLT2 inhibitors have shown promise in improving liver histology and metabolic parameters. Drugs modulating FXR, PPAR, and FGF21 pathways (e.g., obeticholic acid, lanifibranor, pegbelfermin) are in late-stage clinical trials. Anti-inflammatory and antifibrotic strategies represent additional avenues. Noninvasive biomarkers and advanced imaging modalities continue to improve risk assessment and monitoring. Integration of digital health tools and personalized medicine approaches is anticipated to enhance management.

Guideline Recommendations

Recent guidelines from EASL, AASLD, and APASL emphasize early identification of at-risk individuals, primarily those with T2DM, obesity, or metabolic syndrome. Routine screening in high-risk populations, risk stratification using noninvasive scores, and targeted use of imaging are advocated. Lifestyle modification remains first-line therapy, with pharmacologic treatment considered for biopsy-proven NASH with fibrosis. Multidisciplinary care, patient education, and ongoing monitoring are endorsed. Guidelines highlight the importance of addressing cardiovascular risk and optimizing management of comorbid conditions.

Conclusion

MASLD is a prevalent and progressive disease with substantial clinical and public health implications. Early diagnosis, primarily in at-risk populations, remains crucial to prevent progression and complications. Lifestyle interventions form the bedrock of therapy, with emerging pharmacologic agents offering hope for patients with advanced disease. Continued research, multidisciplinary collaboration, and adherence to guideline-based care are pivotal in improving outcomes for patients with MASLD.
