Elevated liver enzymes often pose a diagnostic dilemma for clinicians, due to their non-specific nature and wide array of potential etiologies. This article aims to provide a comprehensive approach to decipher this clinical conundrum.
Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) are the most commonly assessed liver enzymes. Their elevation is suggestive of hepatocellular injury. However, the degree of elevation does not necessarily correlate with the severity of liver damage.
Common causes include viral hepatitis, alcohol-related liver disease, non-alcoholic fatty liver disease, and drug-induced liver injury. Less common causes include autoimmune hepatitis, hemochromatosis, Wilson's disease, and alpha-1 antitrypsin deficiency.
A thorough history and physical examination should be the initial step. If the cause is not apparent, a systematic approach should be adopted. This includes ruling out common causes first, followed by screening for less common causes if initial investigations are inconclusive. Imaging studies and liver biopsy may be required in certain situations.
Management largely depends on the underlying cause. Lifestyle modifications and abstinence from alcohol are often recommended. Regular follow-up with liver function tests is essential to monitor response to treatment and progression of disease. In cases of persistent unexplained elevation, referral to a hepatologist may be necessary.
Elevated liver enzymes present a diagnostic challenge due to the broad differential diagnosis. A systematic and comprehensive approach can help clinicians in making an accurate diagnosis and initiating appropriate management. Continuous medical education and staying updated with the latest research are key to overcoming this diagnostic dilemma.
