Interstitial lung diseases (ILDs) encompass a heterogeneous group of disorders characterized by various forms of interstitial lung damage. This article aims to provide an overview of ILDs, their clinical features, diagnostic methods, and treatment options.
ILDs can be idiopathic or secondary to systemic diseases, environmental exposures, or drugs. They are broadly classified into idiopathic interstitial pneumonias, connective tissue disease-associated ILDs, and occupational and environmental ILDs. Idiopathic pulmonary fibrosis is the most common form of idiopathic interstitial pneumonia.
Patients with ILDs typically present with progressive dyspnea and non-productive cough. Physical examination often reveals bibasilar inspiratory crackles. Diagnosis is based on a combination of clinical features, imaging, and histopathological findings. High-resolution computed tomography (HRCT) is the primary imaging modality, revealing characteristic patterns such as reticular abnormalities, ground-glass opacities, and honeycombing.
Treatment of ILDs is largely dependent on the underlying cause. For idiopathic forms, antifibrotic agents like pirfenidone and nintedanib have shown to slow disease progression. In connective tissue disease-associated ILDs, immunosuppressive therapy is often effective. Lung transplantation may be considered in advanced disease stages. The prognosis varies widely, with idiopathic pulmonary fibrosis having a poor prognosis with a median survival of 2-3 years post-diagnosis.
ILDs are a diverse group of disorders with varying etiologies, clinical presentations, and prognoses. Early recognition and accurate diagnosis are crucial to guide treatment and improve patient outcomes. Further research is needed to better understand these complex diseases and develop more effective therapeutic strategies.
