Pulmonary Function Tests (PFTs) are essential diagnostic tools in the field of respiratory medicine, providing a wealth of information about lung function. However, their interpretation can be complex. This article aims to provide a comprehensive guide to interpreting PFTs for healthcare professionals.
Spirometry is the most common of the PFTs. It measures the volume and speed of air that can be inhaled and exhaled. Key parameters include Forced Vital Capacity (FVC), Forced Expiratory Volume in 1 second (FEV1), and their ratio (FEV1/FVC). A reduced FEV1/FVC ratio indicates obstructive lung disease, while a reduced FVC with a normal FEV1/FVC ratio suggests restrictive lung disease.
These tests measure the lungs' ability to transfer gas from inhaled air to the red blood cells. The most common is the Diffusing Capacity for Carbon Monoxide (DLCO). Reduced DLCO may indicate conditions such as emphysema or pulmonary fibrosis.
These tests measure the total volume of air the lungs can hold (Total Lung Capacity, TLC), the volume remaining at the end of a normal exhalation (Functional Residual Capacity, FRC), and the volume of air remaining after a maximal exhalation (Residual Volume, RV). Increased RV and FRC with a normal TLC suggest air trapping, common in obstructive diseases.
These tests assess airway hyperresponsiveness, a hallmark of asthma. A significant decrease in FEV1 following exposure to a bronchoconstrictor indicates a positive test.
Interpreting PFTs is a skill that requires understanding of the tests' physiological basis and clinical implications. This guide provides a foundation for healthcare professionals to accurately interpret these tests, ultimately improving patient care in respiratory medicine.
