Acute chest pain (ACP) is a common symptom that often leads to hospital admission and can signify a broad spectrum of conditions, from benign to life-threatening. The diagnostic evaluation of ACP remains a clinical challenge due to its varied etiologies and presentations. This article aims to provide a comprehensive approach to ACP, aiding clinicians in their diagnostic and therapeutic decision-making.
The initial evaluation of ACP should focus on identifying life-threatening conditions, such as acute coronary syndromes (ACS), aortic dissection, and pulmonary embolism. This stage involves a detailed history, physical examination, and immediate diagnostic tests like electrocardiogram and chest radiographs. High-risk features such as older age, cardiovascular risk factors, and severe or worsening symptoms should prompt immediate referral for further evaluation.
The diagnostic approach to ACP should be systematic and guided by the pre-test probability of different conditions. For patients with a high likelihood of ACS, immediate cardiac biomarker testing and possible coronary angiography should be considered. For others, further testing should be guided by the patient's history, physical examination, and initial test results. The use of scoring systems, such as the HEART score, can aid in risk stratification and guide further testing.
Management of ACP should be tailored to the underlying cause. For ACS, immediate revascularization and medical management with antiplatelet agents and statins are the mainstay of treatment. For non-cardiac causes of ACP, treatment should be directed at the underlying cause. Regardless of the etiology, all patients should receive symptomatic relief, and those with ongoing pain or hemodynamic instability should be closely monitored.
Following initial treatment, all patients with ACP should be evaluated for their risk of future cardiovascular events. This includes assessment of cardiovascular risk factors, lifestyle modification, and consideration of long-term medical therapy. Regular follow-up is essential to monitor symptoms, adherence to therapy, and response to treatment.
ACP presents a diagnostic challenge due to its varied etiologies and presentations. A systematic approach to evaluation, guided by pre-test probabilities and risk scores, can aid in the accurate diagnosis and treatment of ACP. Regardless of the underlying cause, all patients should receive symptomatic relief and be evaluated for their risk of future cardiovascular events. By adopting a comprehensive approach, clinicians can improve outcomes for patients presenting with this common and often perplexing symptom.
