Pulmonary hypertension (PH) is a complex and often misunderstood condition. This article aims to demystify its intricacies, facilitating a comprehensive understanding for healthcare professionals.
PH is defined as an increase in mean pulmonary arterial pressure ≥ 25 mmHg at rest. It's classified into five groups based on etiology, clinical presentation, hemodynamic characteristics, and treatment strategy. These include pulmonary arterial hypertension, PH due to left heart disease, PH due to lung diseases and/or hypoxia, chronic thromboembolic PH, and PH with unclear multifactorial mechanisms.
The pathophysiology of PH is multifactorial, involving vasoconstriction, vascular remodeling, and in situ thrombosis. The endothelial dysfunction leads to an imbalance between vasodilators and vasoconstrictors, pro-thrombotic and anti-thrombotic factors, and cell proliferation and apoptosis. This results in increased pulmonary vascular resistance, right ventricular overload, and eventually right heart failure.
PH often presents with nonspecific symptoms such as dyspnea, fatigue, and syncope, making early diagnosis challenging. Diagnosis is confirmed through right heart catheterization, but echocardiography serves as a useful screening tool. Additional diagnostic tests include pulmonary function tests, six-minute walk test, and cardiac MRI.
Treatment strategies for PH are group-specific and aim to improve symptoms, slow disease progression, and enhance quality of life. General measures include oxygen therapy, diuretics, and anticoagulation. Specific therapies include endothelin receptor antagonists, phosphodiesterase-5 inhibitors, and prostacyclin analogs. In advanced cases, lung transplantation may be considered.
Understanding the complexities of PH is vital for its effective management. Early diagnosis, accurate classification, and appropriate treatment are key to improving patient outcomes. As healthcare professionals, it is crucial to stay abreast of the latest developments in PH to provide the best possible care to our patients.
