Unexplained weight loss (UWL), defined as a loss of more than 5% of body weight over 6-12 months, can be an indicator of a serious underlying condition. For clinicians, diagnosing the cause can be challenging due to the wide range of potential etiologies.
A comprehensive history and physical examination are paramount in the evaluation of UWL. This should include detailed dietary history, lifestyle factors, medication use, and psychosocial history. The physical examination should focus on signs of malnutrition, systemic diseases, and malignancy.
While there is no standard diagnostic algorithm for UWL, initial laboratory tests should include complete blood count, metabolic panel, thyroid function tests, HIV test, and tumor markers. Imaging studies, such as CT scan or PET, may be required based on specific clinical suspicion.
Given the multifactorial nature of UWL, a multidisciplinary approach involving primary care physicians, gastroenterologists, endocrinologists, psychiatrists, and dieticians can be beneficial in reaching a definitive diagnosis.
While common causes such as malignancies, gastrointestinal disorders, and psychiatric conditions account for most cases of UWL, rare causes like Addison's disease, celiac disease, and parasitic infections should also be considered in the differential diagnosis.
Diagnosing UWL requires a systematic and comprehensive approach. While the diagnostic process can be complex and time-consuming, it is crucial to identify and address the underlying cause promptly to prevent further health deterioration. Continued research and development of more targeted diagnostic tools will aid in the timely diagnosis and management of patients presenting with UWL.
