A thorough physical examination, coupled with targeted investigations such as chest X-rays, spirometry, and bronchoprovocation tests, can aid in the identification of less common etiologies.
Empirical therapy based on the most likely diagnosis can be an effective initial approach. For instance, proton pump inhibitors for suspected GERD, or inhaled corticosteroids for NAEB. However, this approach requires careful patient follow-up to assess response and adjust treatment as necessary.
Emerging diagnostic tools, such as bronchial thermoplasty and impulse oscillometry, hold promise for the future. These tools can provide more precise information about airway inflammation and reactivity, potentially leading to more targeted treatments.
Ultimately, treatment should be individualized, addressing the underlying causes and taking into account patient preferences. This may involve pharmacological treatments, lifestyle modifications, or a combination of both. Refractory cases may benefit from multidisciplinary input, including pulmonology, gastroenterology, and otolaryngology expertise.
Chronic cough is a complex condition that requires a comprehensive diagnostic and therapeutic approach. By staying abreast of emerging diagnostic tools and maintaining a patient-centered focus, physicians can effectively manage this common, yet often challenging, condition.
