Unexplained weight loss (UWL) is a clinical conundrum often encountered in practice. This phenomenon, defined as a loss of more than 5% of body weight over a period of 6-12 months without a known cause, can be indicative of underlying serious illnesses. A systematic and comprehensive approach to its evaluation is crucial to ensure timely diagnosis and intervention.
The first step is a thorough history and physical examination. Key aspects to investigate include dietary habits, medication use, psychosocial factors, and symptoms suggestive of systemic diseases. A comprehensive review of systems can help identify potential causes such as malignancy, endocrine disorders, gastrointestinal diseases, and psychiatric illnesses.
Following the initial evaluation, laboratory investigations should be tailored based on the findings. Basic tests include complete blood count, metabolic panel, thyroid function tests, and inflammatory markers. Imaging studies and endoscopies may be warranted in some cases. If these investigations fail to reveal a cause, a more extensive workup, including tests for rare diseases, may be considered.
Management of UWL hinges on treating the underlying cause. In cases where no cause is identified, a watchful waiting approach may be adopted, with periodic reassessments. Nutritional support should be provided to all patients, and those with significant weight loss may require referral to a dietitian.
Evaluating unexplained weight loss requires a systematic and comprehensive approach. While it can be challenging, understanding the potential underlying causes and maintaining a high index of suspicion for serious illnesses is essential. A meticulous history, physical examination, and judicious use of investigations can facilitate diagnosis and timely intervention, ultimately improving patient outcomes.
