Acute chest pain (ACP) is a common, yet complex, symptom presenting in clinical practice. It represents a broad spectrum of conditions, from benign to life-threatening. A comprehensive approach to its evaluation and management is vital.
Initial evaluation should focus on identifying high-risk features suggestive of acute coronary syndrome (ACS). History, physical examination, and ECG are fundamental. Risk stratification tools, such as the HEART score, can guide further management. Biomarkers, primarily high-sensitivity troponins, provide additional diagnostic clarity.
Management depends on the underlying cause. In suspected ACS, immediate reperfusion therapy is crucial. For non-ACS ACP, treatment should target the specific etiology. It's essential to consider non-cardiac causes, including gastrointestinal, pulmonary, and musculoskeletal conditions.
Education about symptom recognition and timely presentation is crucial. Patients should understand the importance of seeking immediate medical attention for potential cardiac symptoms.
Preventive measures, including lifestyle modifications and pharmacotherapy, play a significant role in reducing recurrent chest pain and adverse cardiac events. Regular follow-up ensures optimal medical therapy and risk factor control.
ACP is a challenging clinical scenario requiring a comprehensive and systematic approach. Prompt evaluation, appropriate management, patient education, and preventive measures are key to improving outcomes. As clinicians, we must remain vigilant and adaptable, ensuring our practice reflects the evolving evidence in this critical area of medicine.
