With the growing burden of tuberculosis (TB) worldwide, the need for rapid and accurate diagnostic tools has never been more critical. This article provides an overview of the recent advancements in TB detection, focusing on contemporary diagnostic approaches.
Modern molecular diagnostic tests have revolutionized TB detection. These include nucleic acid amplification tests (NAATs), such as Xpert MTB/RIF and Xpert MTB/RIF Ultra. These tests offer rapid results, high sensitivity and specificity, and simultaneous detection of resistance to rifampicin, a first-line anti-TB drug. The World Health Organization (WHO) recommends these tests as the initial diagnostic approach in adults and children suspected of having TB.
Next-generation sequencing (NGS) is emerging as a promising tool for TB diagnosis and drug resistance detection. It enables comprehensive analysis of the entire Mycobacterium tuberculosis genome, providing insights into disease epidemiology, transmission dynamics, and drug resistance mechanisms. However, the utility of NGS in routine clinical practice is currently limited by its high cost and technical complexity.
Immunological tests, such as the interferon-gamma release assays (IGRAs) and tuberculin skin test (TST), remain useful tools for TB detection, particularly in latent TB infection. These tests measure the immune response to TB antigens. Despite their limitations, including cross-reactivity with non-tuberculous mycobacteria and the BCG vaccine, they continue to play a crucial role in TB control programs.
In conclusion, advancements in TB detection are paving the way towards more rapid, accurate, and comprehensive diagnosis. Molecular diagnostic tests and NGS offer promising avenues, while immunological tests continue to be integral. However, the optimal use of these tools requires ongoing research, investment in health infrastructure, and capacity building to address the global TB burden effectively.
