Pyrexia is a physiological response to a variety of stimuli, most commonly infection. It is mediated by pyrogens, which reset the hypothalamic temperature set-point, leading to an increase in body temperature. Understanding the underlying pathophysiology can help determine the cause and guide treatment.
A systematic approach is essential in diagnosing pyrexia. A detailed history, thorough physical examination, and targeted investigations are pivotal. The history should focus on potential sources of infection, recent travel, medication use, and systemic symptoms. Laboratory investigations should be guided by the clinical suspicion.
The management of pyrexia primarily involves treating the underlying cause. Antipyretics can be used to provide symptomatic relief, but they do not address the root cause. In cases where the cause remains unidentified despite extensive evaluation, empirical treatment may be initiated based on the most likely differential diagnoses.
