Enteric fever, commonly known as typhoid fever, is a potentially fatal multisystemic illness primarily caused by Salmonella Typhi and Salmonella Paratyphi. Despite advancements in healthcare, it remains a significant public health concern, especially in developing countries. This article aims to review recent advancements in the diagnosis and treatment strategies for enteric fever.
Early and accurate diagnosis of enteric fever is crucial for effective management. Traditional methods like blood culture have limitations such as time consumption and low sensitivity. Recent advancements include molecular techniques like PCR (Polymerase Chain Reaction) and Nucleic Acid Amplification Tests (NAATs) that offer high sensitivity and specificity. Additionally, serological tests such as Typhidot and Tubex TF have shown promise in early detection.
Antibiotic therapy remains the cornerstone of enteric fever treatment. However, the emergence of multidrug-resistant (MDR) and extensively drug-resistant (XDR) strains poses a significant challenge. Therefore, the focus has shifted towards newer antibiotics like azithromycin and third-generation cephalosporins. Moreover, the use of fluoroquinolones, despite resistance concerns, still plays a role in managing uncomplicated cases.
Prevention is always better than cure. In this regard, the development and implementation of effective vaccines such as Vi polysaccharide vaccine (Typhim Vi) and typhoid conjugate vaccine (Typbar-TCV) have shown significant potential in controlling the spread of enteric fever. These vaccines have demonstrated good safety profiles and high efficacy rates.
In conclusion, the management of enteric fever has seen significant advancements in recent years. Diagnostic techniques have evolved to provide quicker and more accurate results. Treatment strategies have adapted to the challenge of antibiotic resistance, and vaccines have been developed to prevent the disease. However, continuous research and development is essential to stay ahead of the evolving pathogen and ensure effective control of enteric fever.
