Osteoporosis, a systemic skeletal disease characterized by low bone mass and microarchitectural deterioration, is a significant public health concern. With the aging population, the prevalence of osteoporosis is expected to increase. This article provides an overview of the advancements in the management of osteoporosis, focusing on recent therapeutic options and diagnostic tools.
New imaging techniques have revolutionized the diagnosis of osteoporosis. High-resolution peripheral quantitative computed tomography (HR-pQCT) provides detailed images of the bone microarchitecture, enhancing the prediction of fracture risk. Trabecular bone score (TBS), a texture parameter that evaluates pixel grey-level variations in lumbar spine dual-energy X-ray absorptiometry (DXA) images, is another promising tool that provides additional information about bone quality.
Recent years have witnessed the emergence of novel pharmacological treatments. Anti-sclerostin antibodies, such as romosozumab, increase bone formation and decrease bone resorption, showing a significant reduction in vertebral and non-vertebral fractures. Anabolic agents like Teriparatide and Abaloparatide stimulate bone formation, offering another therapeutic avenue. Moreover, Denosumab, a RANKL inhibitor, has demonstrated superior efficacy in increasing bone mineral density (BMD) and reducing fracture risk.
Non-pharmacological interventions remain integral to osteoporosis management. These include lifestyle modifications such as adequate calcium and vitamin D intake, regular weight-bearing exercise, smoking cessation, and limiting alcohol intake. Furthermore, fall prevention strategies are vital in reducing fracture risk in this population.
The landscape of osteoporosis management is rapidly evolving, with advancements in both diagnostic tools and therapeutic options. These innovations promise a more comprehensive and personalized approach to osteoporosis care. Healthcare professionals need to stay updated with these advancements to optimize patient outcomes and reduce the burden of this debilitating disease.
