The history should focus on the onset, duration, frequency, and characteristics of the stool, and any associated symptoms. Physical examination should assess hydration status, abdominal tenderness, and signs of systemic disease. Laboratory tests, including stool studies, blood tests, and imaging studies, may be required based on the initial evaluation.
Management of chronic diarrhea is primarily aimed at treating the underlying cause. Dietary modifications, including lactose-free or gluten-free diets, may be beneficial in some patients. Pharmacological therapy, such as antimotility agents, bile acid binders, and anti-inflammatory drugs, may be indicated based on the etiology. In refractory cases, referral to a gastroenterologist may be necessary.
Chronic diarrhea is a complex condition with a multifactorial etiology, requiring a comprehensive approach for its evaluation and management. A thorough clinical evaluation, combined with targeted diagnostic testing, can help identify the underlying cause. Management should be tailored to the individual patient, focusing on treating the underlying cause and alleviating symptoms.
