Peptic ulcer disease (PUD) continues to be a prevalent gastrointestinal disorder that poses a significant burden on healthcare systems worldwide. Recent advancements in diagnosis, treatment, and management have transformed the approach towards PUD, contributing to improved patient outcomes.
Modern diagnostic tools have revolutionized PUD management. Endoscopy, the gold standard, now benefits from high-definition imaging and narrow-band imaging, enhancing the detection of ulcers and precancerous lesions. Non-invasive tests such as the urea breath test, stool antigen test, and serologic tests have improved the identification of Helicobacter pylori, the leading cause of PUD.
Pharmacotherapy remains at the forefront of PUD management. Proton pump inhibitors (PPIs) continue to be the mainstay, with newer agents providing longer durations of acid suppression and fewer side effects. Antibiotic regimens have also evolved, with quadruple therapy now recommended for H. pylori eradication due to increasing antibiotic resistance. Moreover, the development of gastroprotective agents, such as rebamipide and ecabet, offers additional therapeutic options.
While pharmacotherapy is effective, refractory and complicated PUD may necessitate surgical or endoscopic intervention. Innovations in endoscopic techniques, including endoscopic submucosal dissection and endoscopic mucosal resection, have improved the management of complicated PUD, reducing the need for surgery. However, laparoscopic and robotic surgery have also advanced, offering minimally invasive alternatives when necessary.
Prevention remains key in PUD management. Strategies include H. pylori eradication, avoidance of NSAIDs, and patient education about lifestyle modifications. Telemedicine has emerged as a useful tool for patient education, enabling healthcare professionals to provide personalized advice and monitor adherence to treatment remotely.
In summary, advancements in the management of PUD, from enhanced diagnostic tools to innovative therapeutic strategies, have significantly improved patient outcomes. Continued research and innovation are essential to further optimize PUD management and reduce its global burden.
