Osteoporosis, a systemic skeletal disease characterized by reduced bone mass and microarchitectural deterioration, is a major public health issue. To optimize the management of osteoporosis, healthcare professionals need a comprehensive understanding of the disease and its management.
Osteoporosis is often silent until a fracture occurs. It is predominantly seen in postmenopausal women but can affect men and premenopausal women. Risk factors include age, family history, low body mass index, and lifestyle factors such as smoking and excessive alcohol consumption. Early identification and management can significantly reduce the risk of fractures.
Diagnosis is based on bone mineral density (BMD) measured by dual-energy x-ray absorptiometry (DXA). The World Health Organisation defines osteoporosis as a BMD 2.5 standard deviations below the young adult mean. Fracture risk assessment tools, such as FRAX, can also help identify high-risk individuals.
Management of osteoporosis involves lifestyle modifications, adequate calcium and vitamin D intake, and pharmacological therapy. First-line treatment options include bisphosphonates, denosumab, and selective estrogen receptor modulators. Anabolic agents like teriparatide and abaloparatide are used in severe cases. Treatment choice depends on the patient's risk profile and potential side effects.
Regular follow-up and monitoring are essential. BMD should be re-evaluated every 1-2 years. Treatment effectiveness can be assessed through changes in BMD, biochemical markers of bone turnover, and fracture incidence. Adherence to therapy is crucial for treatment success and should be regularly assessed.
Optimizing osteoporosis management requires a comprehensive approach, including understanding the disease, risk assessment, appropriate treatment, and diligent follow-up. By staying informed about the latest evidence and guidelines, healthcare professionals can contribute significantly to reducing the burden of this debilitating disease.
