Vertigo, a symptom rather than a condition, continues to be an enigma for many healthcare professionals. The complexity of its pathophysiology and the diversity of its presentations often pose diagnostic challenges. This article aims to provide an overview of vertigo and suggest a systematic clinical approach for its management.
Vertigo is a subjective sensation of movement, often described as a feeling of spinning or falling. It is typically a manifestation of vestibular dysfunction, which may be peripheral or central in origin. Peripheral vertigo, the more common type, usually results from disorders of the inner ear or vestibular nerve. Central vertigo, on the other hand, arises from pathologies in the brainstem or cerebellum.
A thorough history taking is the cornerstone of diagnosing vertigo. The onset, duration, and triggers of vertigo, associated symptoms such as tinnitus or hearing loss, and any neurological signs should be meticulously noted. Physical examination should focus on vestibular and neurological assessments. Bedside tests like the Dix-Hallpike maneuver and the head impulse test can provide valuable diagnostic clues.
Depending on the clinical suspicion, appropriate investigations such as audiometry, vestibular function tests, or neuroimaging may be required. Management of vertigo primarily involves treating the underlying cause. Symptomatic relief can be provided through vestibular suppressants, while vestibular rehabilitation therapy can help improve balance and coordination.
Vertigo, despite its common occurrence, often remains poorly understood. A comprehensive clinical approach, encompassing diligent history taking, focused physical examination, and judicious use of investigations, can significantly enhance the diagnostic accuracy. As healthcare professionals, we must strive to demystify vertigo, ensuring optimal patient care and improving the quality of life for those affected.
