Chronic cough can be attributed to a multitude of factors such as upper airway cough syndrome, gastroesophageal reflux disease, and non-asthmatic eosinophilic bronchitis. Understanding the pathophysiology of these conditions is crucial to formulating an effective treatment plan.
Diagnosis of chronic cough should be systematic and comprehensive, beginning with a thorough history and physical examination. The use of objective tools like chest radiographs, spirometry, and bronchoprovocation tests can provide valuable insights. If initial investigations are inconclusive, consider more specialized tests such as high-resolution computed tomography or bronchoscopy.
Treatment should be etiology-specific and patient-centered. Empirical therapy for the most common causes may be initiated, but if the cough persists, targeted therapy based on diagnostic tests should be considered. Non-pharmacological measures, including behavioral therapy and physiotherapy, can also play a crucial role in management.
Despite advances in understanding and managing chronic cough, challenges persist. These include the lack of standardized diagnostic criteria and the paucity of effective treatments for refractory cases. Future research should focus on developing novel diagnostic tools and therapeutic agents, as well as exploring the role of personalized medicine in managing chronic cough.
Chronic cough is a complex condition that requires a comprehensive and systematic approach to diagnosis and treatment. By understanding its etiology and pathophysiology, employing a thorough diagnostic process, and utilizing targeted treatment strategies, clinicians can effectively manage this condition and significantly improve patients' quality of life.
