A detailed history, encompassing the nature, location, onset, duration, and associated symptoms of the pain, is crucial. Special attention should be given to factors that exacerbate or alleviate the pain, as well as any relevant past medical or surgical history.
Physical examination should be systematic, starting with inspection and palpation, followed by auscultation and percussion. The presence of scars, distension, or visible peristalsis provides valuable clues. Tenderness, guarding, or rigidity may indicate underlying pathology, while specific signs can suggest certain diagnoses.
Basic laboratory tests, including complete blood count, liver and renal function tests, and inflammatory markers, can guide further investigations. Imaging studies, such as ultrasound or computed tomography, are often necessary to confirm the diagnosis. More specialized tests, like endoscopy or colonoscopy, may be required based on clinical suspicion.
Management should be tailored to the underlying cause. Analgesia, while often necessary, should not compromise the clinical assessment. Patients with non-specific abdominal pain may require reassessment or further investigations if symptoms persist. A clear follow-up plan is essential to ensure appropriate care.
Abdominal pain evaluation requires a methodical approach, incorporating a thorough history, physical examination, and judicious use of investigations. Recognizing 'red flags', understanding the limitations of physical findings and investigations, and tailoring management to the individual patient are key to successful outcomes in this challenging area of clinical practice.
