Unexplained weight loss (UWL), defined as a loss of more than 5% of body weight over 6 to 12 months, is a common yet challenging clinical presentation. The etiology can range from benign to life-threatening conditions, making a structured approach to evaluation crucial.
The first step involves a comprehensive history and physical examination. This should focus on dietary history, medication use, psychosocial factors, and symptoms suggestive of systemic diseases. Particular attention should be paid to signs of malignancy, endocrine disorders, gastrointestinal diseases, and psychiatric illnesses.
If the initial assessment is inconclusive, routine laboratory tests including complete blood count, metabolic panel, liver function tests, thyroid function tests, and inflammatory markers should be considered. Imaging studies such as chest X-ray or abdominal ultrasound may be indicated based on the clinical suspicion.
In cases where routine investigations fail to identify the cause, more specialized testing may be warranted. Endoscopy, CT or MRI scans, and specific hormonal assays may be considered based on the patient's symptoms and risk factors.
Management of UWL should be directed towards the underlying cause. In cases where no specific cause can be identified, symptomatic and supportive treatment should be provided. Nutritional support and regular follow-up are essential in all cases.
In conclusion, UWL is a complex clinical issue that requires a systematic and comprehensive approach. While it can be a marker of serious illness, a significant proportion of cases remain unexplained even after extensive evaluation. This underscores the importance of a thorough initial assessment, judicious use of diagnostic tests, and a patient-centered approach to management.
