Chronic cough, defined as a cough lasting more than eight weeks, is a common presentation in clinical practice. Despite its prevalence, the pathophysiology is often complex and multifactorial, making it a challenging condition to manage effectively.
Chronic cough can be attributed to various etiologies, including but not limited to asthma, gastroesophageal reflux disease (GERD), and upper airway cough syndrome (UACS). An intricate interplay between these conditions often exists, further complicating the diagnostic process. Understanding the underlying pathophysiology is crucial in formulating an effective treatment plan.
An exhaustive history and physical examination are the cornerstones of diagnosing chronic cough. Special attention should be given to identifying potential triggers and exacerbating factors. Ancillary investigations like chest X-rays, spirometry, and laryngoscopy may aid in confirming the diagnosis and ruling out other conditions.
Treatment should be individualized, targeting the underlying cause(s). Inhaled corticosteroids may be beneficial for asthma-related cough, while proton pump inhibitors (PPIs) are often effective for GERD-induced cough. For UACS, first-generation antihistamines and nasal corticosteroids may be employed. Behavioral interventions, such as smoking cessation and avoidance of known triggers, should be encouraged.
Refractory chronic cough, unresponsive to standard treatment, poses a significant challenge. In such cases, a more extensive workup may be required, including high-resolution computed tomography (HRCT) and bronchoscopy. Novel therapeutic options, such as neuromodulators and speech therapy, may also be considered.
Chronic cough is a complex condition requiring a comprehensive, multifaceted approach for effective management. By understanding the underlying pathophysiology, employing a detailed diagnostic evaluation, and tailoring treatment strategies, clinicians can significantly improve patient outcomes and quality of life.
