Tuberculosis (TB) remains a global health concern, necessitating the need for innovative and accurate diagnostic methods. This article provides an overview of contemporary diagnostic approaches to TB, highlighting their strengths and limitations.
Microscopy and culture have been the gold standard for TB diagnosis for decades. However, these methods are time-consuming and have low sensitivity, particularly in patients with HIV co-infection or paucibacillary disease.
Molecular tests, such as the Xpert MTB/RIF assay, have revolutionized TB diagnosis. These tests provide rapid results, have high sensitivity and specificity, and can detect drug resistance. However, their high cost and need for sophisticated laboratory infrastructure limit their use in resource-constrained settings.
Interferon-gamma release assays (IGRAs) and tuberculin skin tests (TSTs) are commonly used to detect latent TB infection. While these tests are useful, they cannot differentiate between latent and active TB infection, and their results can be influenced by BCG vaccination and non-tuberculous mycobacterial infection.
Imaging techniques, such as chest radiography and computed tomography (CT) scans, are valuable tools for TB diagnosis. They provide visual evidence of disease and can help in the detection of extrapulmonary TB. However, their interpretation requires expertise and they cannot confirm TB diagnosis alone.
While advances have been made in TB diagnostics, no single test is perfect. A combination of methods is often required for accurate diagnosis. Future research should focus on developing affordable, rapid, and highly sensitive tests that can be used at the point-of-care. Such innovations could significantly enhance TB control efforts worldwide.
