Neurological examination and neuroimaging, when indicated, can aid in ruling out secondary causes. The International Classification of Headache Disorders (ICHD) provides a diagnostic framework, helping clinicians categorize headaches into primary and secondary, and further into specific subtypes.
Management strategies should be individualized, considering the type, severity, and impact of the headache on the patient's quality of life. Non-pharmacological approaches, including lifestyle modifications and cognitive-behavioral therapy, are often beneficial. Pharmacological treatment varies depending on the headache type, with abortive therapy for acute episodes and preventive therapy for chronic conditions. Emerging therapies, such as neuromodulation and monoclonal antibodies targeting the calcitonin gene-related peptide pathway, offer promising avenues for refractory cases.
Patients should be educated about their condition, treatment options, and the importance of adherence to therapy. Regular follow-up is crucial to assess treatment response, adjust therapy as needed, and manage potential side effects.
