Parkinson's disease (PD) is a progressive neurodegenerative disorder that primarily affects the motor system. It is characterized by bradykinesia, rigidity, resting tremor, and postural instability. The disease is associated with the degeneration of dopaminergic neurons in the substantia nigra and the presence of Lewy bodies.
The etiology of PD is multifactorial, involving a complex interplay of genetic and environmental factors. The primary pathophysiological feature is the loss of dopaminergic neurons in the substantia nigra pars compacta. This neuronal loss leads to a reduction in striatal dopamine levels, resulting in motor symptoms. The presence of Lewy bodies, composed mainly of alpha-synuclein, is another pathological hallmark.
Patients typically present with asymmetric motor symptoms, including resting tremor, bradykinesia, rigidity, and postural instability. Non-motor symptoms such as autonomic dysfunction, cognitive impairment, and mood disorders may also be present. The diagnosis is primarily clinical, based on the presence of cardinal motor symptoms and response to dopaminergic therapy.
Treatment of PD is symptomatic, aiming to improve quality of life. Pharmacological therapy is the mainstay of treatment, with levodopa being the most effective drug. Other medications include dopamine agonists, MAO-B inhibitors, and COMT inhibitors. In advanced stages, surgical interventions like deep brain stimulation may be considered.
While significant strides have been made in understanding PD, it remains a challenging disease to manage due to its progressive nature and the complexity of its symptomatology. Early recognition and an individualized, multidisciplinary approach to management can significantly improve patient outcomes. As healthcare professionals, continuous education and staying abreast with the latest research is crucial in providing optimal care to these patients.
