Acute chest pain (ACP) is a common symptom encountered in clinical practice, often indicating potentially life-threatening conditions. Accurate diagnosis and management are critical, yet the diverse etiologies make this a complex task.
ACP diagnosis begins with a thorough history and physical examination. Electrocardiography (ECG) and cardiac biomarkers are the initial diagnostic tests. However, their sensitivity and specificity vary. Advanced imaging modalities, such as coronary computed tomography angiography (CCTA), can provide additional diagnostic value. Risk stratification tools, such as the HEART score, can guide further testing and management.
ACP management is driven by the underlying cause. In cases of acute coronary syndrome (ACS), immediate revascularization is crucial. Pharmacologic treatment, including antiplatelet and anticoagulant therapies, is also essential. In contrast, non-cardiac causes of ACP require targeted treatment based on the specific etiology. For example, gastrointestinal causes may necessitate proton pump inhibitors or H2 antagonists, while musculoskeletal pain may respond to nonsteroidal anti-inflammatory drugs.
Patient education is a vital aspect of ACP management. Patients should be informed about the importance of early symptom recognition and the need for immediate medical attention. Furthermore, lifestyle modifications and adherence to prescribed medications contribute significantly to secondary prevention.
In conclusion, ACP is a diagnostic challenge due to its diverse etiologies. A comprehensive approach, incorporating thorough evaluation, appropriate diagnostic testing, targeted management, and patient education, is crucial. Further research is needed to refine our diagnostic tools and therapeutic strategies, ultimately improving patient outcomes in this complex clinical scenario.
