Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, remains a significant health concern worldwide. Early diagnosis and treatment are crucial in reducing morbidity and mortality.
The gold standard for diagnosis is blood culture, providing definitive evidence of infection. However, due to its low sensitivity, other diagnostic methods such as Widal test and PCR have been utilized. Recent advancements include the Typhidot test and Tubex test, offering improved sensitivity and specificity.
Antibiotic therapy remains the cornerstone of treatment. Fluoroquinolones were once the first-line therapy, but resistance has led to a shift towards third-generation cephalosporins and azithromycin. Patient management also involves supportive care, including hydration and fever management.
Vaccination is the most effective preventive measure. Two vaccines, Vi polysaccharide vaccine (ViPS) and Ty21a, are currently available and provide substantial protection.
Effective strategies for enteric fever involve a combination of early diagnosis, appropriate antibiotic therapy, supportive care, and vaccination. Continued research into diagnostic methods and antibiotic resistance is essential.
