Chronic cough, defined as a cough lasting more than eight weeks, presents a diagnostic challenge for clinicians due to its multifactorial etiology. It can significantly impact a patient's quality of life, necessitating a comprehensive approach for accurate diagnosis and effective management.
Diagnosis begins with a thorough patient history and physical examination, focusing on identifying common causes such as gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS). If the cause remains elusive, additional diagnostic tests such as chest imaging, spirometry, or bronchoscopy may be required. Clinicians should also consider less common causes such as non-asthmatic eosinophilic bronchitis, or even psychogenic cough.
Management strategies should be tailored to the underlying cause. For GERD-related cough, proton pump inhibitors can be beneficial, while inhaled corticosteroids may be effective for asthma-related cough. Antihistamines and nasal steroids may alleviate UACS-associated cough. Refractory cases may necessitate a multidisciplinary approach involving pulmonologists, gastroenterologists, and even psychologists.
Emerging therapies such as P2X3 receptor antagonists show promise in treating refractory chronic cough. Furthermore, cognitive behavioral therapy (CBT) has shown efficacy in managing psychogenic cough. These advancements underscore the importance of staying abreast with the latest research in this field.
Chronic cough is a complex condition requiring a comprehensive diagnostic approach and individualized management strategy. By considering both common and less common causes, and by staying informed about emerging therapies, clinicians can enhance their ability to effectively diagnose and manage this challenging condition.
