Peptic ulcer disease (PUD), a common gastrointestinal disorder, has witnessed significant advancements in management techniques. This article aims to present a comprehensive overview of these developments, highlighting their practical implications for healthcare professionals.
Improved diagnostic strategies have revolutionized PUD management. High-definition endoscopy, combined with image-enhancement technologies like Narrow Band Imaging, has enhanced the detection of PUD. Moreover, Helicobacter pylori stool antigen tests and urea breath tests have improved the accuracy of H. pylori detection, a leading cause of PUD.
Pharmacological management of PUD has seen significant advancements. Proton pump inhibitors (PPIs) remain the cornerstone of treatment, with novel formulations offering improved bioavailability and duration of action. Additionally, the introduction of vonoprazan, a potassium-competitive acid blocker, has shown superior ulcer healing rates compared to traditional PPIs. The management of H. pylori-associated ulcers has also evolved, with quadruple therapy now recommended as first-line treatment due to increasing antibiotic resistance.
Endoscopic therapeutic techniques have also evolved. Endoscopic submucosal dissection and endoscopic full-thickness resection have emerged as effective techniques for the management of gastric ulcers, reducing the need for invasive surgical interventions. Moreover, endoscopic hemostasis techniques, such as the use of hemoclips and argon plasma coagulation, have significantly improved the management of bleeding peptic ulcers.
The management of PUD has significantly evolved, with advancements in diagnostic tools, pharmacotherapy, and endoscopic techniques. These developments have enhanced the detection, treatment, and management of PUD, leading to improved patient outcomes. As healthcare professionals, staying abreast of these advancements is crucial to providing optimal patient care in the management of PUD.
