Peptic ulcer disease (PUD) remains a prevalent condition worldwide, necessitating an updated understanding of its pathophysiology, diagnosis, and treatment. This article presents a comprehensive approach to PUD, highlighting recent advancements in management and treatment strategies.
While Helicobacter pylori and nonsteroidal anti-inflammatory drugs (NSAIDs) remain the primary etiological agents, recent studies underscore the role of genetic predisposition, stress, and other lifestyle factors. An individualized approach to PUD, considering these multifactorial causes, optimizes patient outcomes.
Endoscopy continues to be the gold standard for PUD diagnosis. However, non-invasive tests like urea breath tests, stool antigen tests, and serologic tests for H. pylori have gained prominence due to their convenience and accuracy.
First-line treatment for PUD involves eradicating H. pylori with triple or quadruple therapy, alongside proton pump inhibitors (PPIs). For NSAID-induced ulcers, discontinuation of NSAIDs and PPI therapy are recommended. Recent advancements include the use of vonoprazan, a potassium-competitive acid blocker, showing superior efficacy to PPIs in certain populations. Moreover, the role of probiotics in enhancing H. pylori eradication and reducing side effects of antibiotics is gaining recognition.
Healthcare professionals must educate patients about lifestyle modifications, such as avoiding NSAIDs, limiting alcohol, and cessation of smoking. Emphasizing the importance of adherence to medication regimens is crucial to prevent recurrence.
Managing PUD effectively requires an updated, comprehensive approach considering the latest advancements in pathophysiology, diagnosis, and treatment. An individualized treatment plan, coupled with patient education, can significantly improve patient outcomes and prevent recurrence. As healthcare professionals, our role in staying abreast with these updates and implementing them in practice is vital.
