The presentation of fever with rash is a common yet challenging scenario in clinical practice. This symptom complex can be indicative of a myriad of conditions ranging from benign self-limiting diseases to life-threatening emergencies. Hence, a comprehensive and systematic approach is crucial in managing such cases, ensuring accurate diagnosis and appropriate treatment.
Various infectious, autoimmune, neoplastic, and drug-related causes can present as fever with rash. Detailed history taking, including travel, exposure, medication, and past medical history, is critical in narrowing down the differential diagnoses. Infections, such as viral exanthems, bacterial endocarditis, and tick-borne diseases, often top the list. Autoimmune diseases like systemic lupus erythematosus and vasculitis can also present similarly. Adverse drug reactions, malignancies, and factitious disorders should also be considered.
A thorough physical examination, focusing on the characteristics and distribution of the rash, systemic signs, and organ-specific symptoms, can provide valuable clues. Laboratory investigations, including complete blood count, inflammatory markers, serology, and skin biopsy, may be necessary based on the clinical suspicion. Imaging studies can be useful in cases with systemic involvement. The diagnostic approach should be tailored to the individual patient, keeping in mind the most probable etiologies.
Management should be guided by the underlying diagnosis. For infectious causes, appropriate antimicrobial therapy is indicated. Autoimmune and neoplastic conditions may require immunosuppressive or chemotherapeutic agents, respectively. In the case of drug-induced fever with rash, discontinuation of the offending agent is usually sufficient. Supportive care, including antipyretics and topical treatments for the rash, can provide symptomatic relief.
Fever with rash poses a diagnostic dilemma due to its vast etiological spectrum. A comprehensive approach, incorporating a detailed history, focused physical examination, and judicious use of diagnostic tests, can guide clinicians towards the correct diagnosis. Early recognition and appropriate management can significantly improve patient outcomes in these challenging cases.
