Introduction

Enteric fever, primarily caused by Salmonella Typhi and Salmonella Paratyphi, remains a significant public health concern worldwide. Despite advancements in diagnostic methods and therapeutics, challenges persist in its timely diagnosis and effective treatment. This article aims to provide an updated approach to the management of enteric fever.

Diagnostic Advances

Traditional diagnosis of enteric fever relied on blood cultures, which have a sensitivity of 40-60%. However, recent advancements have led to more accurate diagnostic methods. Molecular techniques such as Polymerase Chain Reaction (PCR) and Nucleic Acid Amplification Tests (NAATs) have demonstrated higher sensitivity and specificity. Immunodiagnostic tests, such as Typhidot and Tubex TF, are also emerging as rapid and reliable diagnostic tools.

Treatment Strategies

The treatment of enteric fever has been complicated by the emergence of multi-drug resistant (MDR) strains. Empirical therapy, therefore, should be guided by local antibiograms. Fluoroquinolones, once the first-line treatment, have seen reduced efficacy due to resistant strains. Third-generation cephalosporins, such as ceftriaxone and cefixime, are now often preferred. Azithromycin has also shown promise, particularly in areas with high fluoroquinolone resistance. In severe cases, combination therapy may be required.

Vaccination and Prevention

Vaccination remains a crucial component of enteric fever prevention. Currently, two vaccines (Ty21a and Vi polysaccharide) are available and have shown moderate efficacy. However, the recent introduction of a conjugate Vi vaccine (TCV) offers a significant improvement, providing longer-lasting and more robust immunity.

Conclusion

Enteric fever continues to pose considerable challenges to healthcare professionals due to evolving pathogen resistance and diagnostic limitations. However, advancements in molecular diagnostics, the emergence of effective antibiotics, and the introduction of more efficient vaccines provide hope for improved management. Continuous research and surveillance are crucial to ensure the timely diagnosis and effective treatment of this persistent public health concern.
