Common causes include alcohol consumption, nonalcoholic fatty liver disease, viral hepatitis, medications, and metabolic disorders. Identifying the cause requires a careful history, physical examination, and appropriate diagnostic tests.
Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) are the most commonly evaluated liver enzymes. Mild elevations may indicate chronic diseases like hepatitis C or hemochromatosis, while high levels suggest acute liver injury. Alkaline phosphatase (ALP) and gamma-glutamyl transferase (GGT) elevations suggest biliary obstruction or liver infiltration.
A stepwise approach is recommended. Initial tests should include serologic markers for viral hepatitis, iron studies, and imaging. If these are inconclusive, advanced tests like elastography or liver biopsy may be necessary.
Management should be directed towards the underlying cause. Monitoring liver enzyme levels can help assess response to treatment and disease progression. Regular follow-up is essential, particularly in patients with chronic liver diseases.
