Diabetes mellitus, a chronic metabolic disorder, has been linked with an increased risk of cardiovascular disease (CVD). This relationship, although well-established, is complex and multifaceted, warranting further exploration.
Hyperglycemia, a hallmark of diabetes, triggers a cascade of pathophysiological changes that contribute to vascular damage. These include increased oxidative stress, inflammation, and endothelial dysfunction, which are key players in atherogenesis. Furthermore, insulin resistance, common in type 2 diabetes, exacerbates these changes and promotes atherogenic dyslipidemia.
Diabetic patients often exhibit a cluster of cardiovascular risk factors. Hypertension, obesity, and dyslipidemia frequently coexist with diabetes, enhancing the cardiovascular risk. The co-occurrence of these conditions, termed metabolic syndrome, increases the risk of CVD more than any single factor.
Several studies have demonstrated the benefits of tight glycemic control in reducing CVD risk among diabetic patients. However, the optimal level of glycemic control is a topic of ongoing debate, with some studies suggesting potential harm from overly aggressive glucose lowering.
Given the high cardiovascular risk in diabetes, risk stratification is crucial. Several risk stratification tools, incorporating diverse risk factors, have been developed for this purpose. However, their application in clinical practice remains inconsistent.
In conclusion, diabetes significantly increases the risk of CVD through multiple pathways. A comprehensive understanding of this relationship is essential for effective cardiovascular risk management in diabetic patients. Future research should focus on refining risk stratification tools and identifying the optimal glycemic target to reduce cardiovascular risk without causing harm.
