Vertigo, characterized by a sensation of feeling off balance, is a prevalent clinical symptom often reported in primary care settings. It is associated with a variety of conditions, primarily those affecting the inner ear or brain. This article aims to provide a comprehensive clinical approach to vertigo for healthcare professionals.
Vertigo is not a disease but a symptom of various underlying pathologies. It is often described as a spinning sensation or a feeling of loss of balance. These symptoms may be accompanied by nausea, vomiting, or difficulty walking. The most common causes of vertigo are benign paroxysmal positional vertigo (BPPV), Meniere's disease, and vestibular neuritis.
When evaluating a patient with vertigo, a detailed history and physical examination are crucial. The history should include onset, duration, severity, triggers, and associated symptoms. The physical examination should include a thorough neurological and otologic examination. Special tests like the Dix-Hallpike maneuver or the Romberg test can be utilized to further characterize the vertigo.
Management of vertigo depends on the underlying cause. For BPPV, canalith repositioning maneuvers are the treatment of choice. In cases of Meniere's disease, a combination of medications, dietary modifications, and sometimes surgery is required. Vestibular neuritis is usually managed with corticosteroids and vestibular rehabilitation therapy.
Vertigo is a complex symptom with diverse underlying causes. A systematic approach to history taking, examination, and investigation can help in identifying the cause and formulating an appropriate management plan. As healthcare professionals, understanding the intricacies of vertigo can significantly improve patient outcomes.
