Chronic cough, defined as a cough lasting more than eight weeks, is a common yet challenging condition encountered in clinical practice. Despite its prevalence, the diagnosis and management of chronic cough can be complex, often leading to frustration for both the patient and the physician. This article aims to provide a comprehensive approach to diagnosing and managing chronic cough, helping to unravel the enigma that it often presents.
The first step in managing chronic cough involves a thorough diagnostic evaluation. This includes a detailed history and physical examination, supplemented by targeted investigations. The focus should be on identifying the common causes such as gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS). However, it's crucial to consider less common etiologies like lung cancer, sarcoidosis, or foreign body aspiration, especially when the cough is associated with other alarming symptoms.
Once the cause is identified, a targeted treatment plan should be initiated. For GERD-related cough, lifestyle modifications and proton pump inhibitors are the mainstay of treatment. Inhaled corticosteroids are effective for asthma-induced cough, while antihistamines and decongestants are usually beneficial for UACS. For patients with unexplained chronic cough, empirical treatment based on the most likely cause might be a reasonable approach. However, if the cough persists despite these measures, referral to a specialist may be required.
It's important to educate patients about the nature and course of chronic cough. This can help alleviate anxiety and enhance adherence to treatment. Patients should also be advised to return for follow-up if the cough persists or worsens, or if new symptoms develop.
