The fight against Tuberculosis (TB) remains a critical global health challenge. Despite advancements in healthcare, early diagnosis remains a hurdle. This article aims to explore the contemporary diagnostic approaches available in healthcare today.
The conventional method for TB diagnosis is the sputum smear microscopy. It is cost-effective and provides rapid results. However, its sensitivity is limited, especially in HIV co-infected patients and children. The Culture method, although more sensitive, is time-consuming and requires sophisticated laboratories, limiting its use in resource-limited settings.
GeneXpert MTB/RIF, a real-time polymerase chain reaction (PCR) based test, has revolutionized TB diagnosis. It provides results within hours and has high sensitivity and specificity. It also detects rifampicin resistance, aiding in the management of drug-resistant TB. Nevertheless, its high cost and need for regular calibration make it less feasible for low-income countries.
Tuberculin skin test (TST) and Interferon Gamma Release Assays (IGRAs) are immunological tests used to detect latent TB infection. TST is inexpensive and easy to perform but has low specificity in BCG-vaccinated individuals. IGRAs, although more specific, are expensive and require a well-equipped laboratory.
Radiology plays a crucial role in TB diagnosis. Chest radiography can detect pulmonary TB, while CT and MRI are useful in diagnosing extrapulmonary TB. However, imaging cannot confirm TB and needs to be complemented by microbiological or histological confirmation.
In conclusion, while significant strides have been made in TB diagnosis, challenges persist. The ideal diagnostic tool should be sensitive, specific, rapid, affordable, and easy to use. Hence, there is a pressing need for continuous research and innovation to develop such a tool, especially for use in resource-limited settings.
