As healthcare professionals, we often encounter gastrointestinal bleeding (GIB) in our practice. The management of this condition requires a comprehensive understanding of its etiology, diagnostic modalities, and therapeutic options.
GIB can be classified into upper and lower based on its origin. Peptic ulcer disease, esophageal varices, and gastritis are common causes of upper GIB, while diverticulosis, colorectal cancer, and angiodysplasia often lead to lower GIB. Identifying the source is crucial for effective treatment.
Endoscopy remains the gold standard for diagnosing GIB. Upper endoscopy is usually performed first, except in cases with obvious signs of lower bleeding. Colonoscopy and capsule endoscopy are also valuable tools. Imaging studies and angiography can be used when endoscopic methods are inconclusive or contraindicated.
Initial management includes hemodynamic stabilization and risk stratification. Endoscopic therapy is the first-line treatment for most causes of GIB. Pharmacological therapy, such as proton pump inhibitors for peptic ulcers and vasoactive drugs for variceal bleeding, is also essential. Surgery or interventional radiology may be required for refractory or recurrent bleeding.
Preventive measures, such as the use of nonsteroidal anti-inflammatory drugs and anticoagulants, should be reviewed. Follow-up endoscopy is often recommended to ensure healing and prevent rebleeding. Long-term management strategies should be personalized based on the patient's condition and risk factors.
Managing GIB requires a comprehensive approach, from understanding its etiology to implementing appropriate treatment and preventive measures. As healthcare professionals, we must stay updated on the latest evidence to provide the best care for our patients.
