Gestational diabetes mellitus (GDM) is a common metabolic disorder that affects pregnant women. It is associated with an increased risk of maternal and neonatal morbidity, making its effective management crucial for optimizing outcomes. This guide aims to provide healthcare professionals with a comprehensive approach towards GDM management.
Early detection of GDM is paramount in preventing adverse outcomes. It is recommended to screen all pregnant women between 24-28 weeks of gestation using an oral glucose tolerance test. High-risk women should be screened as early as the first prenatal visit.
Medical Nutrition Therapy (MNT) is the cornerstone of GDM management. It involves the intake of a balanced diet, maintaining regular meal timings, and monitoring carbohydrate intake. MNT should be individualized according to the patient's nutritional needs and lifestyle.
Regular physical activity is beneficial in managing GDM. It aids in glucose regulation and reduces insulin resistance. Moderate-intensity exercises, such as brisk walking, are recommended for at least 30 minutes a day, most days of the week.
If MNT and physical activity fail to achieve glycemic control, pharmacological intervention becomes necessary. Insulin therapy is considered the gold standard, but oral hypoglycemic agents like metformin and glyburide are also options.
Women with GDM are at risk of developing type 2 diabetes postpartum. Therefore, it is essential to conduct a 75g oral glucose tolerance test at 6-12 weeks postpartum and continue regular follow-ups to monitor glucose levels.
Managing GDM effectively requires a comprehensive approach, including early detection, lifestyle modifications, pharmacological management, and postpartum follow-up. By adhering to these guidelines, healthcare professionals can optimize outcomes for both the mother and the newborn.
