Peptic ulcer disease (PUD) is a common gastrointestinal disorder characterized by the erosion of the gastrointestinal mucosa resulting from digestive action of gastric acid and pepsin. With advances in understanding the pathophysiology and etiology of PUD, management strategies have significantly evolved.
Diagnostic techniques have become more refined. Endoscopy, while invasive, remains the gold standard for diagnosis and allows for direct visual assessment of the ulcer and biopsy for H. pylori detection. Non-invasive tests, such as urea breath test and stool antigen test, have also gained popularity due to their high sensitivity and specificity.
Pharmacological management has seen considerable advancements. Proton pump inhibitors (PPIs) remain the cornerstone of therapy, with newer generations providing more potent and longer-lasting acid suppression. H. pylori eradication therapy has also evolved, with quadruple therapy now recommended as first-line treatment. Novel drugs targeting gastric mucosal protection and acid secretion are also in development.
Surgery for PUD, once common, is now reserved for complications such as perforation, bleeding, and gastric outlet obstruction. Minimally invasive techniques have improved surgical outcomes and decreased hospital stay.
The discovery of H. pylori and its causal relationship with PUD revolutionized management. Eradication of H. pylori not only heals the ulcer but also prevents recurrence, reducing the need for long-term acid suppression.
Advancements in the management of PUD have drastically improved patient outcomes. Improved diagnostic techniques allow for early and accurate diagnosis. Pharmacological advancements have optimized acid suppression and H. pylori eradication, and the role of surgery has become more defined. As we continue to refine our understanding of PUD, we can look forward to further advancements in its management.
