Initial evaluation of thyroid nodules involves a detailed patient history and physical examination. Risk factors such as radiation exposure, family history of thyroid cancer, and a history of rapidly growing nodules should be carefully considered. Physical examination typically includes palpation of the thyroid to assess the size, consistency, and location of the nodule.
Ultrasound remains the imaging modality of choice for thyroid nodules. It provides valuable information about the size, composition, echogenicity, and vascularity of the nodule, which can help differentiate benign from malignant nodules. Additional imaging modalities like CT or MRI may be needed in complex cases.
Thyroid function tests, including TSH and free T4, are essential in the evaluation process. A suppressed TSH level may indicate a hyperfunctioning nodule, while normal or elevated TSH levels suggest a non-functioning nodule.
Fine-needle aspiration (FNA) biopsy is the most reliable method for differentiating benign from malignant nodules. The decision to perform FNA should be based on the size, sonographic features, and risk factors of the nodule.
Thyroid nodule evaluation requires a multifaceted approach, combining patient history, physical examination, imaging studies, thyroid function tests, and FNA biopsy. This comprehensive approach ensures accurate diagnosis and appropriate management, thereby improving patient outcomes.
