The management of enteric fever, caused by Salmonella Typhi and Paratyphi, has been significantly impacted by the emergence of antibiotic resistance and the development of novel diagnostic techniques and vaccines. This article aims to provide a comprehensive update on the advancements and best practices in the management of this potentially fatal disease.
Antibiotic resistance has complicated the treatment of enteric fever, with multi-drug resistant (MDR) and extensively drug-resistant (XDR) strains emerging globally. Current best practices recommend empiric therapy with third-generation cephalosporins or azithromycin, while awaiting culture and sensitivity results. In regions with high MDR or XDR prevalence, carbapenems may be considered.
Traditional diagnosis of enteric fever involves blood culture, which has limitations in sensitivity and turnaround time. Recent advancements include molecular methods such as PCR and next-generation sequencing, which offer improved sensitivity and specificity, and can also provide valuable information on antibiotic resistance.
Vaccination remains a cornerstone in the prevention of enteric fever. The WHO currently recommends two vaccines: the Vi polysaccharide vaccine and the Ty21a live attenuated vaccine. However, research is ongoing to develop more effective vaccines, including conjugate vaccines that elicit a longer-lasting immune response.
Effective management of enteric fever also involves public health measures such as improving sanitation, providing access to clean water, and educating the public on hygiene practices. Rapid identification and treatment of cases, along with contact tracing, are also crucial in controlling outbreaks.
While challenges remain in the management of enteric fever, advancements in treatment strategies, diagnostic techniques, and vaccine development provide hope for improved patient outcomes. Continued research, coupled with strong public health measures, is key to controlling this global health threat.
