Non-alcoholic fatty liver disease (NAFLD) is a significant global health concern, affecting approximately 25% of the global population. It is characterized by an accumulation of fat in the liver, not due to excessive alcohol consumption. The disease spectrum ranges from simple steatosis to non-alcoholic steatohepatitis (NASH), potentially progressing to cirrhosis and hepatocellular carcinoma.
NAFLD is the most common cause of chronic liver disease worldwide. Its prevalence is increasing in parallel with the obesity and type 2 diabetes epidemics. Risk factors include metabolic syndrome components, such as obesity, insulin resistance, dyslipidemia, and hypertension. NAFLD is often asymptomatic, leading to underdiagnosis and delayed treatment.
The pathogenesis of NAFLD is multifactorial and complex, involving genetic and environmental factors. The 'two-hit' hypothesis has been widely accepted, where the first 'hit' involves hepatic lipid accumulation, and the second 'hit' involves oxidative stress and inflammation. However, recent research suggests a 'multiple-hit' model, including gut microbiota changes, insulin resistance, genetics, and epigenetics.
Management of NAFLD focuses on reducing risk factors, primarily through lifestyle modifications. Weight loss, healthy diet, and regular exercise are recommended. Pharmacological treatment is considered for patients with NASH or fibrosis. Emerging therapies targeting various pathogenic pathways are under investigation. Early detection and proactive management are crucial to prevent disease progression.
NAFLD is a prevalent and complex disease with significant public health implications. Understanding its pathogenesis and risk factors is essential for healthcare professionals to facilitate early detection and implement proactive management strategies. Continued research is needed to develop targeted therapies and improve patient outcomes.
