Vertigo, a common clinical symptom characterized by the perception of motion where none exists, can be a diagnostic challenge for healthcare professionals. Understanding the pathophysiology and clinical approach to vertigo is paramount to ensure effective patient management.
Vertigo is primarily a manifestation of vestibular dysfunction. It arises from an imbalance in the neuronal activity of the bilateral vestibular nuclei. This can be due to peripheral causes like benign paroxysmal positional vertigo (BPPV), Meniere's disease, or vestibular neuritis, or central causes such as cerebrovascular disease or migraines.
The clinical approach to vertigo begins with a detailed history and physical examination. The temporal pattern of vertigo, associated symptoms like tinnitus or hearing loss, and provoking factors should be thoroughly evaluated. Physical examination should include Dix-Hallpike and head impulse tests.
Basic investigations include complete blood counts, electrolytes, and imaging studies. More specific tests like electronystagmography (ENG), videonystagmography (VNG), or magnetic resonance imaging (MRI) may be necessary based on the clinical suspicion. Audiometric studies can be useful in cases of suspected Meniere's disease.
Management of vertigo depends on the underlying cause. BPPV can be managed with canalith repositioning maneuvers while vestibular suppressants are used for vestibular neuritis. For Meniere's disease, a combination of dietary modification and diuretics is often effective. In refractory cases, vestibular rehabilitation therapy or surgery may be considered.
Vertigo is a complex symptom that requires a comprehensive clinical approach for accurate diagnosis and effective management. A clear understanding of its pathophysiology, clinical presentation, diagnostic investigations, and management strategies will enhance the ability of healthcare professionals to provide optimal patient care.
