Chronic cough, defined as a cough lasting more than eight weeks, is a common and often challenging condition to manage. Despite comprehensive guidelines, diagnosing and treating chronic cough can be an enigma due to its multifactorial etiology and the complexity of its pathophysiology.
The pathophysiology of chronic cough is multifaceted, involving hypersensitivity and hyperresponsiveness of the cough reflex. A heightened cough reflex sensitivity can be triggered by various factors such as gastroesophageal reflux disease (GERD), postnasal drip syndrome, and asthma. Understanding the underlying pathophysiology is crucial for identifying potential treatment targets.
A systematic approach is essential in diagnosing chronic cough. Detailed history taking and physical examination should be the first steps, followed by chest radiography to rule out common causes. If these initial investigations are unremarkable, further diagnostic tests such as spirometry, bronchoprovocation tests, or high-resolution computed tomography may be required. Importantly, the clinician should be mindful of less common causes of chronic cough, such as lung cancer, tuberculosis, or interstitial lung diseases.
Treatment strategies should be individualized based on the underlying cause. For cough due to GERD, proton pump inhibitors may be beneficial. Inhaled corticosteroids are the treatment of choice for cough-variant asthma, while antihistamines and decongestants may be helpful for postnasal drip syndrome. If a specific cause cannot be identified, empirical treatment with cough suppressants may be considered. However, the long-term use of these medications should be avoided due to potential side effects.
In conclusion, a comprehensive approach to diagnosing and treating chronic cough involves understanding the complex pathophysiology, adopting a systematic diagnostic approach, and individualizing treatment based on the underlying cause. Despite the challenges, the successful management of chronic cough can significantly improve patients' quality of life.
