Introduction

Chronic cough, defined as a cough persisting for more than eight weeks, is a common and often challenging condition to manage in clinical practice. It can significantly impact a patient's quality of life and often poses a diagnostic conundrum due to its multifactorial etiology.

Diagnosing Chronic Cough

A thorough patient history and physical examination are crucial in the initial evaluation of chronic cough. This should be complemented by targeted investigations based on clinical suspicion. Common causes include gastroesophageal reflux disease (GERD), upper airway cough syndrome (UACS) and non-asthmatic eosinophilic bronchitis (NAEB). However, in up to 25% of cases, the cause remains unexplained, leading to the diagnosis of 'chronic idiopathic cough'.

Management Strategies

Management of chronic cough should be etiology-specific. Empirical treatment trials for GERD, UACS and NAEB can be initiated in primary care settings. Refractory cases may benefit from referral to a specialist cough clinic where a multidisciplinary approach, including speech and language therapy, can be employed. Recent advances in our understanding of the pathophysiology of chronic cough have led to the development of novel therapeutic agents such as P2X3 antagonists, which show promising results in early clinical trials.

Challenges and Future Directions

Despite these advancements, chronic cough remains a complex condition to diagnose and manage. A significant proportion of patients do not respond to conventional treatment strategies, highlighting the need for further research in this field. Future directions may include the identification of novel cough biomarkers and the development of personalized medicine approaches based on individual cough phenotypes.

Conclusion

In conclusion, chronic cough is a multifaceted condition requiring a comprehensive diagnostic approach and tailored management strategies. While significant progress has been made, there is still much to be learned about this common and often debilitating condition. As our understanding evolves, it is hoped that we will be able to provide more effective and individualized treatments for our patients suffering from chronic cough.
