Unexplained weight loss (UWL) presents a diagnostic challenge due to its multifactorial etiology. This article aims to provide a comprehensive approach to diagnosing UWL in patients.
UWL can be an initial symptom of a variety of diseases. Commonly, it is associated with malignancies, gastrointestinal disorders, endocrine diseases, and psychiatric conditions. However, in approximately 15-25% of cases, the cause remains unknown.
A thorough clinical assessment is crucial. It includes a detailed history, focusing on dietary intake, physical activity, alcohol and drug use, and symptoms of systemic diseases. The physical examination should concentrate on signs of cachexia, wasting syndromes, and systemic diseases.
Basic laboratory tests, including complete blood count, metabolic panel, liver function tests, thyroid function tests, and urinalysis, can provide valuable clues. If these tests are inconclusive, more specific investigations such as imaging studies, endoscopy, and biopsy may be warranted. The decision to proceed with these tests should be guided by the patient's history and physical examination findings.
Management of UWL should be directed towards treating the underlying cause, if identified. In cases where no cause is found, a symptom-oriented approach, including nutritional support and counseling, may be beneficial.
Diagnosing UWL requires a meticulous, stepwise approach. It is essential to consider all potential causes, perform a comprehensive clinical assessment, and use diagnostic tests judiciously. Despite these efforts, the cause of UWL may remain elusive in some patients. In such cases, a symptom-oriented approach can help manage the patient's condition and improve their quality of life.
