Vertigo, a common clinical condition characterized by a sensation of spinning or feeling off-balance, often presents a diagnostic challenge to healthcare professionals. This article aims to provide a comprehensive clinical approach to diagnosing and managing vertigo.
Vertigo is typically a manifestation of vestibular dysfunction. It can be broadly classified as either peripheral or central, with the former being more common. A careful history and physical examination, including the Dix-Hallpike maneuver and the head impulse test, can help differentiate between the two.
Initial evaluation should aim to identify any red flags such as new-onset headache, diplopia, or ataxia that may suggest a central cause. Neuroimaging, preferably with MRI, should be considered in these cases. Peripheral vertigo can often be diagnosed clinically and is usually benign. However, recurrent episodes may warrant vestibular testing or referral to an otolaryngologist.
Treatment of vertigo primarily involves managing the underlying cause. For peripheral vertigo, maneuvers such as the Epley or Semont can be effective in repositioning displaced otoliths in benign paroxysmal positional vertigo (BPPV). Medications like meclizine can provide symptomatic relief. In Meniere's disease, a low salt diet and diuretics can help control symptoms. Vestibular rehabilitation can also be beneficial in chronic cases.
Central vertigo, though less common, is more concerning due to its association with serious conditions like stroke or multiple sclerosis. Management involves treating the underlying condition and may require a multidisciplinary approach involving neurologists, physiotherapists, and sometimes, psychologists.
Vertigo, while often benign, can be a symptom of serious underlying conditions. A systematic approach to diagnosis, including a thorough history, examination, and appropriate investigations, is crucial. Treatment should be tailored to the underlying cause and may involve a combination of medications, physical maneuvers, dietary modifications, and rehabilitation.
