Despite significant strides in global health, Tuberculosis (TB) remains a leading cause of death worldwide. Early detection is critical in managing the disease and reducing its spread. This article reviews the latest advancements in TB detection and their implications for medical practice.
Traditionally, TB has been diagnosed through sputum smear microscopy, tuberculin skin test, and chest X-rays. However, these methods have limitations such as low sensitivity, inability to differentiate between latent and active TB, and dependency on radiological expertise.
Recent years have seen significant advancements in molecular diagnostics for TB. The WHO-endorsed Xpert MTB/RIF assay, a fully automated nucleic acid amplification test, has shown high sensitivity and specificity for TB and rifampicin resistance. Furthermore, the introduction of the Xpert MTB/RIF Ultra has enhanced diagnostic accuracy, particularly in paucibacillary forms of TB.
IGRAs, another major advancement, measure the immune response to TB-specific antigens. These assays have higher specificity than the tuberculin skin test and are unaffected by prior BCG vaccination. However, they cannot differentiate between latent and active TB, limiting their utility in some scenarios.
Next-generation sequencing (NGS) holds promise for TB detection, offering comprehensive genetic information that can inform treatment decisions. Additionally, the discovery of new biomarkers, such as lipoarabinomannan (LAM) in urine, is paving the way for non-invasive, point-of-care testing.
While traditional diagnostic methods remain in use, advancements in molecular diagnostics, IGRAs, NGS, and biomarker discovery are revolutionizing TB detection. These developments offer increased diagnostic accuracy, rapid results, and potential for personalized treatment strategies. As our understanding of TB evolves, so too will our ability to detect and manage this global health challenge.
